The Technologies for Improving Post-Acute Care Transitions (“Tech4Impact”) grant program is limited to a State agency or instrument of the State (e.g., State Unit on Aging, State Disability Agencies, State Medicaid Agency) that meets specific requirements.
All Tech4Impact Letters of Intent are due July 30, 2010 and must be submitted via www.grants.gov as an attachment to an “Option D” application. See pages 76 and 121-124 for details.

The Center for Technology and Aging (the Center) is pleased to announce its Technologies for Improving Post-Acute Care Transitions grants program (“Tech4Impact”). CTA is partnering with the Centers for Medicaid and Medicare Services (CMS) and the Administration on Aging (AoA) to improve post-acute care transitions and reduce hospital readmissions. The Tech4Impact grant is being offered by CTA as a funding opportunity that complements the AoA/CMS ADRC Evidence-based Care Transitions Programs (“Option D”) grant. Both grants are part of a multi-component funding opportunity called “Implementing the Affordable Care Act to make it easier for Individuals to Navigate their Health and Long-Term Care through Statewide Systems of Information, Counseling and Access.”
CTA is soliciting letters of intent from States that are interested in expanding use of technologies that improve transitions of care from hospital to home and other settings. Total funding available for this grant program is $500,000. Up to six grants will be awarded. The 12-month projects are expected to commence in January 2011.
Tech4Impact funds are intended for older adults or persons with disabilities participating in, or who could benefit from, an evidence-based care transitions program (e.g., Care Transitions Interventions, Transitional Care Model, and Geriatric Resources for Assessment and Care of Elders). CTA will fund community-based initiatives that are led by Aging and Disabilities Resource Centers (ADRC) in selected states. Total funding available for the Tech4IMPACT Diffusion Grants Program is $500,000.
The Center expects to award five to six grants. Grants will be awarded up to $100,000 per grant for a period of 12-months.
